    Description: The management of traumatic dental injuries (TDI) often poses a clinical challenge that affects dentist, patients and their families. Treatment of TDI sequalae can be complex, time consuming, expensive and often requires multidisciplinary collaboration. Through a series of clinical case presentations, this session will show the role of the endodontist in the diagnosis and management of complications of TDI through incorporation of the dental trauma guidelines, contemporary literature and clinical experience.

    Learning Objectives:  

    • Articulate how to utilize the dental trauma guidelines and how to make modifications on a case by case basis
    • Outline endodontic complications following dental trauma and their management
    • Explain the importance of interdisciplinary collaboration in the treatment of complications following TDI

    Isabel Mello, D.D.S., M.Sc., Ph.D.

    Dr. Mello received her DDS degree from the Federal University of Alfenas, Brazil in 1996. In 2000, she completed her residency and Master of Sciences in Endodontics at the University of Sao Paulo, and in 2008, she defended her PhD in Dental Sciences in a joint program between the University of Sao Paulo and the University of British Columbia. She is the Division Head of Endodontics at Dalhousie University in Nova Scotia, Canada. Dr. Mello is an active researcher and has published several papers in peer review journals. She is a board certified Endodontist and a fellow of the Royal College of Dentists of Canada. She serves as a reviewer for the Journal of Endodontics. She serves as consulting staff in the Dentistry Department at the IWK children’s hospital in Nova Scotia and maintains a part-time practice in Dartmouth, Nova Scotia.

    Description: Dental trauma often leads to long term side effects on the pulp chamber and the periapical tissues. This course will go over the long term side effects caused by trauma such as cervical root resorption, internal resorption and root ankylosis. We will explore the treatment options that are sometimes needed several years after the initial dental trauma.

    • Describe what can occur to traumatized teeth over the years
    • Describe how to evaluate and treat teeth with cervical root resorption
    • Explain how to evaluate and treat teeth with root ankylosis

    Peter Z.Tawil, DDS, MS

    After starting his professional path in Mechanical Engineering at McGill University, Dr Tawil decided to pursue his interest in Dentistry. He obtained his DMD at the Université de Montréal and his AEGD certificate at the University of Rochester. Through his dental journey, Dr Tawil discovered his passion for saving the natural dentition and completed his Masters in Endodontics at UNC in Chapel Hill. Upon completion of his studies, Dr Tawil worked as a specialist in private practice for numerous years in Québec and in North Carolina. Furthermore, he started an academic appointment at the University of North Carolina in 2011 where he progressed to become the Olmsted Family Distinguished Professor and the Director for Graduate Endodontics. He has several publications with an emphasis on Periapical Micro-surgery. In addition, Dr Tawil has a devotion for service, he co-founded the Montreal Homeless Street Youth Dental Clinic and has been committed to his ongoing dental mission work in underdeveloped nations.

    Description: It is paramount for all endodontists to have some understating on how to diagnose and treat the most common dental injuries, especially the emergency phase of the treatment. This is because the time from injury to treatment can differentiate between saving a tooth or losing it to later complications. Recently, the AAE has updated its recommendation of emergency treatment and follow up schedule as well as when and how to endodontically treat the most common dental, injuries. In the first part of the webinar there will a detailed description of diagnostic procedures needed in case of oral and dental trauma. Recommended radiographic, facio-skeletal and oral evaluations will be explored. The second part of the webinar will focus on changes in treatment recommendations from the last Guidelines and the rationale behind those changes.

    Learning Objectives: 

    • Explain the updated AAE’s Recommended Guidelines for the Treatment of Traumatic Dental Injuries. 
    • Describe the recommended emergency treatment of most common dental injuries.
    • Explain the need for follow-up and possible late complications.

    Asgeir Sigurdsson, D.D.S., M.S.

    DDS from University of Iceland. Graduated from University of North Carolina (UNC) in 1992, with certificate in endodontics and MS. Full time faculty at UNC 1992-2004, was the Graduate Program Director of Endodontics 1997 to 2004. From 2004 to 2012 in a private endodontic practice in Reykjavik, Iceland and London, UK and Honorary Clinical Teacher in Endodontology, UCL Eastman Dental Institute, UK. In 2012, he became the Chairman and tenured Professor at NYU Dentistry, Department of Endodontics. Additionally, he is an adjunct Professor at UNC. Dr. Sigurdsson is past President of the International Association for Dental Traumatology and 2014 he was appointed as a Director of the American Board of Endodontics and served as the President of the Board 2019-20. He is currently on the editorial board of Dental Traumatology. Dr. Sigurdsson has been invited numerous times to lecture in 47 countries all over the world, on the topics of pain, endodontics, dental trauma and/or forensic sciences. He has written or co-written over 85 per-reviewed articles and 23 textbook chapters on same topics.

    Description: 

    Avulsions is considered to be one of the most severe and complex dental injuries with regards to treatment and long-term prognosis. The extra-alveolar dry period and storage medium in which a tooth is kept prior to replantation remain critical factors affecting the survival and repair potential of the periodontal ligament in avulsed teeth. When replantation is not immediate, replacement root resorption (RRR) and ankylosis is the most common complication. This lecture will discuss the recently published guidelines and present emerging approaches for the treatment of avulsed teeth with long extra-alveolar dry time.

    At the conclusion of this presentation, participants will be able to:         

    • Discuss avulsion injuries and its complications in growing patients.
    • Explain the new published protocols and strategies for avulsed teeth
    • Describe new concepts for rehabilitation and regeneration of injured tissues.

    Nestor Cohenca, D.D.S., F.I.A.D.T.

    Dr. Nestor Cohenca completed the endodontic program at the Hebrew University in Jerusalem cum laude and received the Best Graduate Student Award. He then served 11 years on faculty at the school while maintaining a private practice limited to endodontics. Dr. Cohenca joined the University of Washington and served as tenured professor of endodontics and pediatric dentistry from 2005-2014. Currently he serves as professor and director of endodontics and the Multidisciplinary Traumatology Unit at Seattle Children’s Hospital and maintains a private practice limited to endodontics in Kirkland, Wash. He is a Diplomate of the Israel Board of Endodontics and the American Board of Endodontics. Dr. Cohenca is a Fellow and past-president of the International Association of Dental Traumatology and received an honorary membership to Omicron Kappa Upsilon National Dental Honor Society. He has published more than 80 peer-reviewed articles, 10 chapters and a book. Dr. Cohenca provided more than 250 lectures around the world and is well known as one of the experts in dental traumatology, endo-pedo-related topics, vital pulp therapy, CBCT and root canal disinfection.

    Description: 

    The treatment of dental alveolar trauma should be a multidisciplinary discussion. Early mixed-dentition injuries ranging from simple displacements to advanced intrusions and avulsions will be described in detail with special emphasis on the role of each team member. Proper treatment of injuries in the developing dentition will many times prevent or lessen long-term recurrent problems related to periodontal and restorative disfigurement. Similar injuries in the adult dentition will be differentiated. Implant placement combined with hard and soft tissue management and restoration, will be demonstrated. Participants will be enlightened by the stable and esthetic results achievable when the focus is always from a multidisciplinary approach in the esthetic zone.

    Roger Wise, D.D.S.

    Dr. Wise received his Doctor of Dental Surgery degree from Emory University in 1971. He went on to obtain his Certificate in Periodontics from Boston University in 1973, and then completed his Certificate in Orthodontics in 1977, also at Boston University. He is a Diplomate of the American Board of Periodontology.

    Dr. Wise is a past faculty member and lecturer of the Departments of Orthodontics and Periodontics at Harvard School of Dental Medicine, from 1993 to 2010. He currently lectures at the Department of Periodontics at Harvard University. He has presented more than 200 lectures in the US and throughout the world on the interrelationship between Adult Orthodontics, Periodontics, and Restorative Dentistry. He has also written four textbook chapters and has authored numerous papers in peer-reviewed journals. He is one of the small number of periodontists in the world fully trained in both Periodontics and Orthodontics. In addition to his private practice focus on Periodontics, Implantology, and Orthodontics, Dr. Wise served 15 years as a consultant for the American Journal of Orthodontics and Dentofacial Orthopedics.

    Description: Although different techniques have been suggested for clinical crown lengthening, all of them have some limitations in terms of aesthetics. In aesthetic zones, crown lengthening maneuvers require a complex diagnostic process, special attention should be taken, or the surgical technique could often result in gingival line asymmetry. The basic goal of the surgical extrusion treatment is to coronally reposition the remnant tooth structure, which will guarantee a reliable sound and healthy supragingival tooth support, to offer a good restorative outcome. This technique can be used successfully to treat severely damaged teeth, especially in the anterior aesthetic zone. During the lecture, a simplified protocol for surgical extrusion will be presented, as well as long-term follow-up clinical cases (more than 9 years follow-up period). The advantages and disadvantages of this procedure will be analyzed.

    • Describe and perform the simplified surgical extrusion technique presented during the lecture

    Jenner O. Argueta, D.D.S., M.Sc.

    Dr. Jenner Argueta earned his dentistry and endodontics master’s degree at San Carlos de Guatemala University. He obtained the International Endodontics Program Certificate at Buffalo, The State University of New York. Jenner Is the former president of the Guatemalan Endodontics Academy, period 2016 – 2020, certified researcher at the Guatemalan National Council for Science and Technology and Endodontics professor in post-graduate endodontics programs from different countries in Latin America. His clinical practice is focused on microendodontics and micro-restorative dentistry. 

    Dr. Argueta has multiple academic awards, international scientific publications and, experience as an international lecturer in four continents, he has been lecturing in the most important endodontics congresses in the world (AAE, IFEA, SELA, FOLA, FOCAP, ROOTS, Harvard University, among others). Dr. Argueta is in charge of the social dental program provided by a non-profit organization in Guatemala called ADIZ. ADIZ provide dental attention to students with scholarships sponsored by international organizations.

    Description:Traumatic dental injuries comprise a number of the dental emergency patients that are often seen after hours or on an unscheduled basis in a dental practice environment. While there are a variety of traumatic dental injuries that can occur, each with their own recommended treatment protocols, the initial evaluation and diagnosis of the traumatized dentition makes up a critical aspect of the management of these cases. Through a series of clinical case presentations involving horizontal root fractures, avulsions, luxations and crown fractures along with literature references, this lecture will provide an overview of initial decisions and treatment principles that can lead to successful management of various types of traumatic dental injuries.

      Perform an organized and thorough evaluation of the traumatized dentition.
    • Discuss the value of and limitations of various pulpal tests on traumatized teeth.
    • Identify a variety of traumatic dental injuries and have an understanding of what the immediate treatment principles entail for each injury.

    Garry L. Myers, D.D.S.

    Dr. Garry L. Myers currently serves as the graduate endodontic program director at Virginia Commonwealth University (VCU) in Richmond, Virginia. He completed his dental school education at the University of Texas Health Science Center/ San Antonio in 1985. Six years later he completed his endodontic residency program at Wilford Hall Medical Center at Lackland AFB in San Antonio. He became a Diplomate of the American Board of Endodontics in 1994. After serving on active duty for 13 years, Dr. Myers left the USAF in 1998 to enter full-time private practice in Olympia, Washington where he practiced for the next 16 years. In the summer of 2014, full time education entered the picture when Dr. Myers accepted a position at VCU where he now works. Dr. Myers has been active in organized dentistry having served as the president of the Washington State Association of Endodontists in 2006-07 and as the President of the American Association of Endodontists in 2017-18. Dr. Myers currently serves as an ABE Director and as the endodontic CODA Commissioner. He has spoken internationally at meetings in Japan, South Korea, San Diego and Guatemala.

    Dr. Judy McIntyre attended the Harvard School of Dental Medicine, where she graduated Magna Cum Laude. After graduation, Dr. McIntyre attended the University of North Carolina at Chapel Hill where she completed her endodontics residency and obtained her master’s. Before dental school graduation, she suggested the creation of the Resident & New Practitioner Committee back in 2003 to Past-President Dr. Samuel Dorn. She has proudly served on many AAE committees and is also proud to have completed the Massachusetts Dental Society Leadership Institute, as well as the ADA Diversity in Leadership Institute.

    Description: The management of dental trauma remains a significant clinical challenge affecting healthcare providers and patients alike. Falls, accidents and sport-related injuries are the most frequent causes of dental trauma. Patients can present with a wide variety of injuries ranging from crown or root fractures, to injuries to the supporting periodontal structure, including luxations and avulsions. Treatment is complex, time-consuming, expensive and requires a multidisciplinary approach.     Course Description: 

    Endodontists should play a pivotal role in the diagnosis and the treatment planning of comprehensive cases. The negative impact of occlusal trauma on the periodontium has been thoroughly investigated. However, little attention has been given to the effects of occlusal trauma on the integrity of the human dental pulp and the surrounding hard tissue structures. This lecture will discuss the less known consequences of occlusal trauma such as accelerated pulpal tissue degeneration and the role of occlusal trauma on external root resorption. The implications of occlusal forces on periapical healing and the survivability of endodontically treated teeth will also be addressed.

    At the conclusion, participants should be able to:

    • Evaluate the typical clinical presentation, signs, symptoms, radiographic features, and the pathophysiological course of traumatic occlusion.
    • Perform systematic analysis of the occlusal trauma and treatment plan the cases on evidence based manner.
    • Discuss the studies that show the negative effect of occlusal trauma on the dental pulp, dentin, and cementum.

    Dr. Mohammed Alharbi is an assistant Professor and a consultant of Endodontic at King Abdulaziz University, Jeddah, Saudi Arabia. He is also an Adjunct assistant professor at The University of Pennsylvania department of endodontics. Dr. Alharbi received his BDS Degree from King Abdulaziz University in 2009 with an Honor Degree. Dr. Alharbi completed his Endodontic postgraduate training at the University of Pennsylvania in 2014 and Doctor of Science in Dentistry (DScD) from the University of Pennsylvania in 2017. He is a Diplomate of the American Board of Endodontics. Dr. Alharbi also maintains a part-time private practice limited to Endodontics in Saudi Arabia.
