Description: When a previously treated tooth presents with signs and/or symptoms of post-treatment disease, the clinician faces one of the most challenging treatment-planning decisions in endodontics: Follow-up, nonsurgical retreatment, endodontic microsurgery, or extraction. This webinar presents a structured, evidence-based framework for navigating that decision. We will review the biological causes of post-treatment apical periodontitis, the clinical and radiographic factors that drive case selection and the comparative outcomes of each treatment option. Through representative clinical cases, attendees will learn to match the right intervention to the right tooth and to communicate the rationale clearly to patients and referring colleagues.

  • Describe the biological causes of post-treatment apical periodontitis, including persistent and secondary intraradicular infection, extraradicular infection, and non-microbial causes. 
  • Evaluate the key clinical, radiographic and patient-related factors that guide treatment planning for a previously treated tooth with post-treatment disease. 
  • Compare the indications, outcomes, and limitations of nonsurgical retreatment, endodontic microsurgery, and extraction with replacement.
