Speaker: Albert Goerig, D.D.S., M.S.|This program will address the key office systems and business elements needed to move the office to the next level in practice success and enjoyment. Eighty percent of the success and profitability of an endodontic practice is due to the systems and well trained and empowered team that allows the doctor to focus 95% of their time on treatment and patient care. When these systems are implemented office drama and stress is reduced, profitability can double even when the doctor works less days.

Speaker: George Bruder, D.M.D.|The decision to treat or extract a tooth in the esthetic zone is one of the more complex decisions that a practitioner must make. It is a test of the team’s knowledge and ability to reach the ultimate goal of an esthetic and functional restoration that will last for years to come. All this must be done with the patient’s emotional and financial status considered. This session will focus on the latest challenges in diagnosing endodontically treated teeth that may require surgery to assist in dental dam isolation, apical surgery and/or intentional replantation. This will be both a “why to” presentation focusing on the decision-making process and a “how to” presentation.

Speaker: Nessrin A. Taha, BDS, GradDipClinDent, DClinDent, PhD|Dental caries continues to be a global problem, with root canal treatment being considered as the conventional management for teeth with inflamed pulps. The current improved understanding of the histopathology and the healing potential of the inflamed pulp, and the use of MTA and hydraulic calcium silicate-based materials, have led to a renewed interest in vital pulp therapy procedures as a minimally invasive treatment approach. This presentation will comprehensively review vital pulp therapy for teeth with deep carious lesions, in terms of treatment planning, selection criteria, clinical procedures, clinical outcomes and patients’ satisfaction.

Authors: Norah Alajlan, BDS, MS, Alonso Carrasco-Labra, DDS, MSc, PhD, Bekir Karabucak, DMD, MS, and Su-Min Lee, DDS, MSD, DScD|Chronic inflammation in irreversible pulpitis leads to heightened sensitivity of nociceptive receptors, resulting in persistent hyperalgesia. This poses significant challenges in achieving effective anesthesia for patients with irreversible pulpitis. Various anesthetic techniques and pharmacological approaches have been employed to enhance the success of local anesthesia. Recently, the preemptive use of anti-inflammatory agents, specifically corticosteroids, has gained attention and shown promising results in randomized controlled trials. This systemic review and meta-analysis aimed to evaluate the impact of systemically administered corticosteroids on enhancing anesthetic success in patients undergoing endodontic treatment.

Speaker: Hal F. Duncan, B.D.S M.Clin.Dent, Ph.D|Caries prevalence remains high throughout the world, with the burden of disease shifting to older and socially disadvantaged groups. If left untreated, caries will advance through dentine stimulating a progressive pulpitis and eventually pulp infection and necrosis; however, if conservatively managed pulpal recovery occurs even in deep carious lesions. As a profession and speciality, we should be striving to develop new biologically-based minimally-invasive solutions; however, to do so, an applied understanding of pulpitis and repair processes is critical. During this presentation we will discuss both the ESE position statement of ‘Deep Caries and the Exposed Pulp’ and also the ESE S3-level clinical guideline recommendations. During this talk we will consider amongst other things: How does the pulp respond to bacterial challenge? How localised is the inflammatory response? Could we consider utilising biomarkers expressed in pulp inflammation for diagnosis or repair?

Speaker: Brooke Blicher, DMD, Certificate in Endodontics|Pain is fraught in the dental world. It is often the driver of emergency dental visits, yet it also keeps many fearful patients out of the dental chair. Thankfully, we live in a world with effective pharmaceuticals to manage pain of endodontic origin pre-operatively, effective anesthesia to eliminate painful delivery of care, and strategies to minimize postoperative discomfort. In this lecture, we’ll present an organized and evidence-based algorithm for both diagnosing and managing pain of endodontic origin, including before, during and following the delivery of definitive care.

Speaker: Qian Xie, BDS, PhD|The goal of root canal treatment is to prevent or reverse apical periodontitis by removal of necrotic/infected or irreversibly inflamed tissues, followed by obturation of the root canal space. Mechanical instrumentation and irrigation play a key role in the removal of pulp tissue, contaminants, debris, and microorganisms from the root canal system. Different adjunctive agitation techniques have been developed to help irrigating solutions penetrate the complexities of the root canal system. Passive ultrasonic irrigation, apical negative pressure irrigation systems, sonic activation, multisonic agitation, and laser activation have been reported to be superior to conventional positive pressure needle irrigation in regards to smear layer removal, antimicrobial effect, and decreased post-operative pain. Minimally invasive canal preparation has been promoted with the activated irrigation approaches. However, what is not known is the effect of minimally invasive instrumentation, in conjunction with an activated irrigation approach on clinical outcomes (retention/longevity, healing of apical periodontitis). Can infected root canals be predictably disinfected following conservative root canal preparation? Based on a review of the current literature and personal clinical experience, appropriate preparation sizes for root canal treatment will be explored.
